We are in-network with some BCBS plans through our partnership with Lactation Consultants of America (LCA).

Covered Plans: This includes nearly all Anthem and Federal (Basic, Standard, & PPO) plans, and some non-Michigan BCBS policies.

Verification Required

To ensure we use the correct intake process (LCA In-Network vs. Self-Pay), we MUST verify your benefits with your insurance company before or soon after scheduling.

Coverage Type	Question to Ask
In-Network (via Lactation Consultants of America)	“Do I have in-network coverage for lactation services under CPT code S9443 (lactation class) with diagnosis Z39.1, billed as professional services (not facility)?”
Out-of-Network (If not covered above)	“Do I have out-of-network benefits for lactation services, and can a superbill be submitted for CPT code 99404 with diagnosis Z39.1?”

Process and Fees

Based on your benefit verification, you will follow one of these two tracks:

1.In-Network Process (Through Lactation Consultants of America)

This is the track to follow if your insurance confirms coverage through LCA.

• Cost: You may have a small co-pay or cost-share (typically $10–$20) per appointment.
• The insurance company’s process through Lactation Consultants of America requires you to complete time-intensive screening and intake forms. These forms are not optional and must be completed to process your claim.

2. Self-Pay (Out-of-Pocket) Option

If your plan is not covered through LCA, or you prefer to seek reimbursement directly, you can self-pay. We will provide a Superbill (detailed receipt) for you to submit to your insurance for potential out-of-network reimbursement.
